Singapore reports first deaths from COVID-19

Updated 29 Aug 2022: Singapore’s COVID-19 fatality rate
Singapore’s Covid-19 case fatality rate is remarkably low. Why?

A friend of mine, another journalist, texts me every few days with the latest coronavirus case and death counts from Singapore. On Monday: “Singapore is now at 28k cases and 22 deaths.” I checked the Johns Hopkins dashboard on Wednesday morning to confirm: 29,364 confirmed Covid-19 cases and still just 22 deaths.

All of us are amateur epidemiologists now, and my friend’s fascination with Singapore is all about the country’s remarkably low case fatality rate. Based on those numbers, just 0.07 percent of people in Singapore who contracted the coronavirus have died from it. Compare that to the United States, where more than 6 percent of confirmed Covid-19 cases have resulted in death.

To look at it a different way, the chart above compares total Covid-19 deaths per million people in both countries. The US is trying to bend the curve. Singapore doesn’t even have a curve to speak of. What’s going on?

COVID-19 - The Omicron Variant

The Technical Advisory Group on SARS-CoV-2 Virus Evolution (TAG-VE) is an independent group of experts that periodically monitors and evaluates the evolution of SARS-CoV-2 and assesses if specific mutations and combinations of mutations alter the behaviour of the virus. The TAG-VE was convened on 26 November 2021 to assess the SARS-CoV-2 variant: B.1.1.529.

The B.1.1.529 variant was first reported to WHO from South Africa on 24 November 2021. The epidemiological situation in South Africa has been characterized by three distinct peaks in reported cases, the latest of which was predominantly the Delta variant. In recent weeks, infections have increased steeply, coinciding with the detection of B.1.1.529 variant. The first known confirmed B.1.1.529 infection was from a specimen collected on 9 November 2021.

This variant has a large number of mutations, some of which are concerning. Preliminary evidence suggests an increased risk of reinfection with this variant, as compared to other VOCs. The number of cases of this variant appears to be increasing in almost all provinces in South Africa. Current SARS-CoV-2 PCR diagnostics continue to detect this variant. Several labs have indicated that for one widely used PCR test, one of the three target genes is not detected (called S gene dropout or S gene target failure) and this test can therefore be used as marker for this variant, pending sequencing confirmation. Using this approach, this variant has been detected at faster rates than previous surges in infection, suggesting that this variant may have a growth advantage.

Why the surge in Covid-19 deaths in S'pore, and what it means for the future
As seen in other countries, most spikes in Covid-19 cases and deaths are in unvaccinated people, says Professor Dale Fisher. PHOTO: ST FILE

Twelve people have died of Covid-19 this month, even as more than three in four people here have been fully vaccinated.

Against this, we had 37 deaths between January last year, when Covid-19 arrived in Singapore, and July 2021. Over that period, more than 64,000 people were diagnosed with the disease. So why has the number of deaths surged, even as vaccination rates here have gone up?

The short answer is the Delta variant. This variant, which is now infecting people in the community here, spreads two to four times more easily, compared with the original wild-type virus. People who are infected with the Delta variant have very much higher viral loads, and vaccines do not work as well against it, though they do still give fairly high levels of protection.

What we know about Covid-19 deaths among younger adults in Singapore

From Monday (Nov 22), fully vaccinated people can dine together in groups of up to five, even if they are not from the same household. The two-person cap on social gatherings will also be raised to five persons, and households may take in up to five distinct visitors a day from Monday.

If they are from different households, they may only dine in pairs. Homeless in the time of Covid-19
Ironically despite there being available spaces from the government and volunteer groups, there are individuals who still prefer to sleep rough

According to a study from last year, there are around 1,000 people living in the streets of Singapore. Homeless people have always been vulnerable, and during the time of a pandemic, when each person is expected to shelter-at-home, there are individuals who still prefer to sleep rough, despite available spaces from the government and volunteer groups.

A recent story from TODAY Online featured several such individuals who are sleeping in the streets during this circuit breaker period. The article quotes Minister for Social and Family Development Desmond Lee as saying recently that there are still 400 available beds in various shelters that answered the call to help house the homeless at this time. One of the people TODAY talked to had difficulty in finding shelter due to his dependency on alcohol, another was an 80-year-old who actually has employment as a dishwasher but who has gotten used to sleeping rough as he has done this for more than two decades. Yet another homeless man, a cleaner who had been estranged from his wife for over a year, had actually been offered shelter at a church. He turned down the offer out of fear he would be brainwashed.

But perhaps the situation of homeless people in Singapore is still relatively better than in other parts of the world. As community transmissions of the coronavirus have been going down steadily—with the Ministry of Health saying this week it’s down to two per day—the homeless in Singapore have a smaller chance of getting infected.

Unvaccinated vs vaccinated Covid-19 deaths in Singapore

Out of 5.5 million people, 500,000 children below the age of 12 are ineligible to receive the vaccine yet. But while they may be carriers of the virus, they are exceedingly unlikely to end up in hospital or suffer serious adverse effects. Among those over the age of 12, 94 per cent have already received their vaccines. Nevertheless, the remaining six per cent translates to a whopping 300,000 people — still an enormous group, which is highly vulnerable to COVID-19.

According to the Ministry of Health (MOH), individuals from among the six per cent of those who are partially or completely unvaccinated comprised 67.5 per cent of deaths in the first three weeks of October.

What’s really sad is that out of 169 deaths reported in that period, 30 were of those who received one dose of the vaccine. This suggests that they got scared some time before they ended up contracting the disease and went to receive their shot, but did not survive long enough to complete the two-dose cycle and develop necessary immunity.

US raises COVID-19 travel alert for Singapore to highest risk level

The United States on Monday (Oct 18) raised its COVID-19 travel advisory level for Singapore to the highest risk category, asking visitors to avoid travel to the country. The US Centers for Disease Control and Prevention (CDC) adjusted its advisory for Singapore by one rung to Level 4, which indicates a “very high level of COVID-19”.

"Because of the current situation in Singapore, even fully vaccinated travellers may be at risk for getting and spreading COVID-19 variants," said CDC. The US' health protection agency said those who must travel to Singapore should be fully vaccinated, and that they should follow the country's recommendations or requirements, including wearing a mask and observing safe distancing measures.

The US Department of State issued a similar notice on Monday, urging people not to travel to Singapore due to COVID-19.

Why vaccinated people dying from Covid-19 doesn't mean the vaccines are ineffective

Former US Secretary of State Colin Powell died on Monday of Covid-19 complications. His family announced that he was fully vaccinated. He was 84 years old, and had multiple myeloma, a blood cancer.

Health officials worry that anti-vaccine activists will seize upon Powell's death to make the claim that vaccines don't work. If you can still die after being vaccinated for Covid-19, what's the point of getting the vaccine?

What's the answer to that question? I discussed it with CNN Medical Analyst Dr. Leana Wen, an emergency physician and professor of health policy and management at the George Washington University Milken Institute School of Public Health. She is also author of a new book, "Lifelines: A Doctor's Journey in the Fight for Public Health."

COVID-19 - To Boost or not to Boost

How long does the protective immunity provided by the COVID-19 vaccines last? Do we need to “boost” immunity for continued protection? Recently, these questions have thrown science and politics into a contentious situation. The Biden administration has strongly recommended booster shots for COVID-19 beginning this month, with the caveat that the Food and Drug Administration (FDA) would have to approve. As a result, significant public and political pressure has been put on the FDA to fast-track approval of the booster. The FDA traditionally proceeds slowly and cautiously with insistence on rigorous supporting data and usually does not respond well to pressure.

This conflict recently led to the resignation of the FDA’s director of the Center for Biologics Evaluation and Research (CBER), Dr. Marion Gruber, and her deputy director, Dr. Philip Krause. Both are capable scientists with years of experience evaluating vaccines.

Unfortunately, the Biden administration appears not to have adhered to its plan to follow the science when making the recommendation for vaccine boosters. If vaccine-induced immunity is indeed waning, it would be prudent to ready the supply chain to administer millions of booster shots. However, if robust immunity remains sufficient to protect from severe disease, then this rush to boost wastes valuable resources and compromises the goal of ending the pandemic. This decision requires rigorous scientific evidence that protection afforded by the vaccine is indeed waning.

Why is Singapore's COVID-19 death rate the world's lowest

Singapore has the lowest coronavirus case fatality count globally, with just 27 deaths among the more than 57,000 people who have been infected with Covid-19 in the Southeast Asian island.

At 0.05 per cent, Singapore's death rate is well below the global average of around 3 per cent, according to data compiled by Reuters from countries that have recorded more than 1,000 cases.

comparison with countries with a similar sized population shows a stark difference - Denmark's death rate is around 3 per cent, while Finland's is around 4 per cent. Further, nobody has died from the disease in Singapore for more than two months, according to its health ministry.

How COVID-19 Deaths Are Counted

As coronavirus has swept through the United States, finding the true number of people who have been infected has been stymied due to lack of testing. Now, official counts of coronavirus deaths are being challenged, too.

In Colorado, for example, a Republican state legislator has accused the state’s public health department of falsely inflating COVID-19 deaths; in Florida, local media have objected to the State Department of Health’s refusal to release medical examiner data to the public, alleging that the state may be underreporting deaths.

The reality is that assigning a cause of death is not always straightforward, even pre-pandemic, and a patchwork of local rules and regulations makes getting valid national data challenging. However, data on excess deaths in the United States over the past several months suggest that COVID-19 deaths are probably being undercounted rather than overcounted.

10th person who had COVID-19 & died but not added to the official toll

A 44-year-old man who died earlier this week from heart disease had COVID-19, the Ministry of Health (MOH) said on Thursday (Jun 11). The Indian national, known as Case 39327, developed chest and epigastric pain on May 28 and sought medical treatment at a general practitioner, MOH said in its press release. On Monday, he was found unconscious at his place of residence and taken to Singapore General Hospital’s emergency department. Two days after his death, which MOH attributed to ischaemic heart disease, he was found to have had COVID-19.

The man is the 10th person who had COVID-19 and died but was not added to the official toll because the death was not caused by the coronavirus. Seven deaths were attributed to heart- or blood-related issues. One death was attributed to cerebral haemorrhage and another to multiple injuries from a fall. MOH did not specify the cause of death for one of the cases, except that it was not related to COVID-19.

In some cases where patients died due to blood clots or heart-related issues, the patients were only confirmed to have had COVID-19 after their deaths. “Only cases where the attending doctor or pathologist attributes the primary or underlying cause of death as due to COVID-19 infection will be added to the COVID-19 death count,” MOH said. "This is consistent with international practice for classifying deaths."

Why 3 positive cases who died from heart issues were not added to Singapore’s official death toll

Not adding COVID-19 patients who died of heart issues instead of the coronavirus to the official death toll is "consistent with international practice", the Ministry of Health (MOH) said. Three confirmed cases have died of heart issues and not of COVID-19 since the start of April. Their causes of death include ischaemic heart disease and heart attack, and their deaths were not included in the official death toll of 20. There have been medical reports linking COVID-19 with heart issues.

MOH said in response to CNA’s queries that it reports all deaths in patients infected with COVID-19. “However, we add to the COVID-19 death count the cases where the attending doctor or pathologist attribute the primary or underlying cause of death as due to COVID-19 infection," the health ministry said.

“This has been our consistent practice, applied for both Singaporeans and foreigners in Singapore. It is consistent with international practice for classifying deaths.”

26 Covid-19 patients have passed away in S'pore so far but why are 6 not considered Covid-19 deaths?

On May 5, the Ministry of Health (MOH) announced that a 44-year-old male Bangladeshi national (Case 16370) had passed away earlier that day. However, his death was not classified as a Covid-19 death, as the cause was stated to be acute myocardial infarction, or a heart attack.

Singapore currently has 20 recorded Covid-19-linked deaths, but there have so far been an additional six Covid-19 patients reported to have died from causes deemed not to be complications from their Covid-19 infection.

Why are they not counted as Covid-19 deaths by the authorities? The answer, perhaps, lies in how Covid-19 deaths are classified according to the World Health Organisation (WHO) and MOH. Singapore's non-Covid-19 deaths:

A Perfect Storm for an Outbreak

“They’ve contained the coronavirus. Here’s how,” read the New York Times headline on a piece about best responses to COVID-19. Alongside Hong Kong and Taiwan, Singapore was held up as a model for the rest of the world.

That was March 13, when Singapore had 200 confirmed cases of COVID-19. It’s a wildly different situation now. As of the time of writing, Singapore has confirmed 11,178 cases of the coronavirus.

The main reason for this skyrocketing figure has been the discovery of the virus spreading like wildfire within dormitories housing migrant workers. Over 80% of the country’s COVID-19 cases are migrant workers living in dormitories. In the dormitory with the largest cluster thus far, over 15% of the population of 13,000 men have tested positive. The resulting scramble to contain this tsunami of infections has highlighted uncomfortable truths about the city-state’s treatment of the men who built it.

Covid-19 circuit breaker to be extended by one month to June 1

The circuit breaker to choke off the spread of the coronavirus will be extended by another month and existing measures will be tightened until May 4, said Prime Minister Lee Hsien Loong in his fourth national address on the situation.

This means that more workplaces will be closed to further reduce the number of workers keeping essential services going. Some hotspots, such as popular wet markets, remain a problem as large groups of people continue to congregate there, Mr Lee said.

While he noted that the circuit breaker measures have been working, he stressed that Singapore cannot be complacent. He said the number of unlinked cases has not come down, which suggests a "hidden reservoir"ofcases in the community.

Singapore enters new phase of community transmission
Local cases close in on imported ones

As coronavirus clusters and cases with no known connections mushroom here, Singapore is facing the threat of the virus spreading uncontrollably, experts warned yesterday.

Whether this potential time bomb is defused or not is now firmly in the hands of each individual in the country, they stressed.

People must decide at this point whether they choose to cooperate and listen to the reminders on personal hygiene, physical distancing and to stay at home unless absolutely necessary, or if they continue to behave irresponsibly especially in public, said Professor Teo Yik Ying, dean of the National University of Singapore's Saw Swee Hock School of Public Health.

Singapore will shut Terminal 2 of the Changi Airport as part of a cost-cutting move due to the COVID-19 pandemic, which has seriously affected the country’s tourism industry.

All operations at the terminal, including retail, will be suspended for 18 months starting May, Transport Minister Khaw Boon Wan reportedly announced in Parliament today. He also hinted at more closures, noting that only one terminal has so far been enough to handle the traffic at the airport.

Airlines operating at the terminal will be transferred to Changi’s remaining three terminals next month. Singapore Airlines will operate from Terminal 3. “We will save on running costs – for the airport operator, retail tenants, airlines and ground handlers,” The Straits Times quoted him as saying.
